Who We Are
JAG Healthcare is an integrated health care provider dedicated to serving small communities throughout the State of Ohio. With proven capabilities including skilled and intermediate care, assisted living, specialized memory care, and hospice services, we offer a statewide continuum that tailors services to each individual while promoting the unique culture of each community. JAG Healthcare maintains close relationships with acute care providers, medical specialists and community leaders so that new programs and cost effective health care options are continuously being added in order to meet the changing needs of those we serve.
What We Do
JAG Healthcare operates skilled nursing facilities across the state of Ohio, specializing in those with 100 or less beds. We provide excellent care for our residents, as evidenced by our strong historical record of State Surveys conducted by the Ohio Department of Health. Our ownership has a “hands on” approach with a lifetime of experience in the industry. We have an established reputation of taking over facilities that are struggling operationally and/or financially and turning them into first-class facilities.

Our Owner
Jim Griffiths has been providing families and their loved ones with skilled nursing and rehabilitation for virtually his entire life. Jim’s parents bought their first nursing center when he was just eleven years old – this quickly became his second home. Jim’s early introduction to caring for others and making a difference in their lives became the cornerstone of the organization’s vision today.
